Non receipt of the card
 
I had travelled by Etihad Airlines flight EY 150 from ORD-AUH on19th June 2015 and my seat number 7C.
I had made on board purchase, sale 4, Device ORS005701, for a total of USD 657.60.
I had also filled up the frequent flyer program as I was specifically informed that I will get credit for my purchase and also for registering by way of miles to my credit.
Till date I have not received my frequent flyer card nor the details of the miles that I earned for my travel to USA and back.
I am not able to see the credit in my JET AIRWAYS FREQUENT FLYER STATEMENT FOR THE TRAVEL MADE TO USA AND BACK.
My JET AIRWAYS FREQUENT FLYER NUMBER IS 103886635.
I request your kind attention to the matter and please send me the FREQUENT FLYER CARD OF ETIHAD AIRWAYS and details of the miles credited/transfered to JET AIRWAYS FREQUENT FLYER ACCOUNT.
I travelled by Business class
